The story kicks off on an island where a lady named, Sloane Maddison tries to run away from someone. She calls the island's owner, Mister Roarke, for help, and he says he'll send assistance. Sadly, Sloane gets caught by the person chasing her. Later, a plane brings several guests to Fantasy Island. Mister Roarke's assistant, Julia, welcomes them.
Among the guests are Melanie Cole, Gwen Olsen, Patrick Sullivan, and stepbrothers JD and Brax Weaver. Julia says their biggest dreams can come true, but they need to unfold naturally. A guy named Damon watches the guests from afar. After introductions, Julia guides them to their fantasies. Mister Roarke then officially welcomes them. JD and Brax are led to a non-stop party where JD enjoys the company of attractive women, and Brax hangs out with good-looking men, also having some weed. While hav
ing a good time, Brax notices what looks like a ghostly figure of a severely burned man. The next morning, Gwen talks with Mister Roarke, and he asks her about her past. Then, she enters her fantasy a restaurant where she turned down a marriage proposal from her ex-boyfriend to pursue her career. In this fantasy, she meets her ex-boyfriend, Alan. Gwen realizes he's a real person in her fantasy and feels sorry for him. Despite that, she enjoys reconnecting with him, and when Alan proposes again,
she happily says yes. Melanie shares her fantasy with Patrick, wanting to get back at her former school bully. Mister Roarke takes her to a basement, revealing Sloane tied up behind a two-way mirror. Initially thinking Sloane is a hologram, Melanie mistreats her. She records a damaging video and shares it on Facebook, only to discover Sloane is actually there. Panicking, Melanie tries to stop further harm. A man resembling Doctor Torture, who terrified Melanie before, prepares to harm Sloane. Me
lanie uses a cattle prod and toilet water to stop him, freeing Sloane. To hide her actions, Melanie pretends to be just another guest. Patrick's fantasy takes him back to a war setting in the 1980s, reuniting with his father who died in action. In this scenario, Patrick gets captured by other soldiers. Meanwhile, JD and Brax play with weapons and explosives in their mansion, unaware they're causing a commotion. Soldiers nearby hear the commotion and engage in a firefight with enemies. JD and Bra
x realize they're in a dangerous man named Kalishov's mansion, and his armed henchmen are approaching. Melanie and Sloane go into a dark forest to escape the island. Doctor Torture attacks them, but Damon, a former private investigator trapped on the island, rescues them by impaling the surgeon with his machete. Damon reveals he was a guest with a fantasy to reunite with his deceased daughter, but it turned into a nightmare. Damon shares that Mister Roarke gave guests magic water to make their f
antasies real. The trio decides to find a phone for help and fills a canteen with the magic water. Looking into the water fountain, they see other guests' fantasies. Sloane wants to reconcile with her husband, while Melanie's fantasy is to torment Sloane. Sloane discovers Melanie lied about being a guest, allowing Melanie to express her anger. Meanwhile, Mister Sullivan approaches Patrick after finding a photo of himself with his son among Patrick's possessions. Patrick reveals he's Mister Sulli
van's son, but Mister Sullivan remains unconvinced Patrick will prevent a foreseen fate. Mister Sullivan faces a life-threatening situation in a hostage rescue mission in Venezuela scheduled for the next day. Gwen wakes up to a surprising reality she's already married to Alan, with a five-year-old daughter named Lila. This isn't what she wanted for her fantasy. Gwen talks to Mister Roarke and Julia to change it. She originally wanted to go back in time and save someone from a fire she caused. Mi
ster Roarke, who fantasizes about reuniting with his deceased wife, is hesitant to change Gwen's fantasy. However, Gwen convinces him, pointing out that he can only see his wife if he allows her to have her desired fantasy. JD and Brax try to deceive thugs searching for money in the mansion by sending them to the garage. Meanwhile, Gwen enters her new fantasy a burning building where she sees JD and Brax making a quick escape. Determined to rescue the victim, she tries to enter the blazing apart
ment. Gwen encounters Patrick as a cop, but he waits for firefighters. She returns inside, loses consciousness, and is found by Julia. Patrick and soldiers reach the hostage situation, revealing it's at the mansion. JD, Brax, and Patrick fight thugs, but they return as zombies. JD and Mister Sullivan tragically die. Melanie, Sloane, and Damon face a zombie Doctor Torture. Damon sacrifices himself by pushing both off a cliff. The women return to the torture room, call for help, and contact Damon'
s associates. As the situation intensifies, the guests confront Mister Roarke and his associates about the twisted fantasies. They discover their fantasies are connected to a person named Nick who seems to wish them harm. Gwen remembers accidentally starting a fire that killed Nick. Patrick was present but didn't help. JD, Nick's roommate, wanted to check on him, but Brax persuaded him to leave. Melanie planned to date Nick that evening but fell ill and canceled. In an attempt to escape, they ra
ce to board a plane, but it's shot down. They suspect Julia might be Nick's grieving mother seeking revenge. To end the ordeal, they decide to destroy Mister Roarke's fountain to stop nightmarish fantasies. In a cave, they face manifestations of their deepest fears. Sloane encounters her bully self, Patrick faces spirits of people he couldn't save, and Gwen envisions Lila killing Alan. Nick's ghost appears, but doesn't recognize Julia. Melanie attacks Patrick, while others reach the fountain to
destroy it with a grenade. Mister Roarke intervenes, revealing Julia was his wife. They realize they are trapped in Melanie's fantasy. Roarke lets Julia go as she succumbs to her illness. Sloane fulfills her fantasy, wishing for Melanie and Nick to be together. Nick's zombie drags Melanie inside the fountain, causing other zombies to vanish. Patrick sacrifices himself by leaping onto the grenade. The next morning, Gwen, Sloane, and Brax prepare to leave. Mister Roarke tells Brax he still has one
fantasy left, wishing for JD to return home alive. The catch is Brax must stay on the island. Brax allows JD to leave with Gwen and Sloane. As they part, Brax shares JD's nickname for him: "Tattoo." Choosing to stay, Brax becomes Mister Roarke's new assistant. In this way, movie ends here.
