hi class today we're talking about an interactive
patient education plan we will go over the issue the patient was struggling with and what was
done to engage him in his health and discuss his success this is a 75-year-old male patient
who is 59 and weighs 230 lb he was diagnosed with typee 2 diabetes about 1 year ago he is
struggling managing his daily blood sugars that are too high and irregular ranging
from 130 to 200 throughout the day and in results he has a high hemoglobin A1c
of 8.2
so he has a definite clinical concern the patient has shared he only
checks his sugar once daily and no more because he doesn't like pricking his
finger multiple times a day to check his blood sugar and he tries to avoid it
not only is he not checking sugars but is giving himself insulin without knowing
the correct dose which is dangerous so this patient could definitely benefit from
some education and Technology interventions um Joe also shared he is somewhat Health literate
but he does
have trouble with some medical jargon the patient um Joe does ask for explanations
of medical terms that he does not understand he knows the names of his medications and their
uses he has a good understanding of his diabetes and he knows what he eats affects his blood sugar
and he knows that high or or uncontrolled sugars lead to other health complications he does
know that eating healthier foods and adding regular exercise into his lifestyle can lead to
control of his diabetes or better ma
nagement he does know how to use a smartphone and he can
download apps he doesn't currently have any health apps pertaining to his chronic condition
and he has mostly put his health into the hands of his providers all of his strengths
already put him in a good position to move forward to better his health even um with
knowing that he has these weaknesses he could benefit from teaching and encouragement
to take control of his health and his diabetes the way we helped this patient
was by int
roducing interactive patient education by using technology interactive
patient Technologies educate patients about diseases procedures treatments and testing
like lab work colonoscopies and mammograms for example and patient education and awareness
of preventative testing is huge for population Health by thwarting unwanted medical issues
it also helps patients make informed decisions with a better understanding and it
increases medication us usage and understanding education through technol
ogy in
turn makes patients more engaged and accountable by taking charge of their health asking
questions and making healthier decisions it overall improves their health and promotes
Healthy Lifestyles interactive education is used on multiple platforms of media and technology
and it increases patient engagement it allows them to better understand their disease and therefore
follow treatments procedures testing and follow medication recommendations made by their providers
it makes them inv
olved educated and improves Health outcomes the two are intertwined somewhat
but both have one goal of improving Patients health I thought this was a nice
display of the effectiveness of patient education and what it entails
written simply there are three things to successful patient education the
use of Technology um no or little medical jargon and patient feedback which
is important um especially to know what is working so three interactive technologies
that would help Joe become more in
formed and engaged with managing his chronic
conditions are the Dexcom G7 with an app the American Diabetes Association
web page and the wellframe app the Dexcom G7 has a wearable body sensor
which is usually placed on the back of the arm and that measures glucose
readings at regular interval intervals repeatedly it consists of three parts the
sensor the transmitter that sends Wireless readings and then the last part which is the
phone or receiver nearby that displays the readings to the p
atient or the user this piece of
technology is a good fit for the patient because he States his health he's Health literate
enough to operate a smartphone which he has and he can use apps so understanding placement
and usage of the sensor is something I thought would be fairly easy for him to understand as
well also because this will replace the need to poke himself to check his sugars daily this
will increase compliance and provide consistent results and along with diabetes education could
overall reduce his daily sugars and his hemoglobin A1c another interactive technology that
I mentioned is the American Diabetes Association web page this web page includes
educational information about each type of diabetes it talks about checking blood sugars
which there's a picture right here from the web page which a patient can just pull up um
there's a series of topics you can go in on the web page um and they have a p actual
PDF file you can download um and it goes over multiple iss
ues or questions that come up
that patients might have um so it also talks about hemoglobin A1c and living with diabetes
it includes health and wellness tips weight management medication information healthy
eating guidelines and tools and resources for crisis situations and caregiver support this
is an all-inclusive education platform to answer any question the patient may have regarding their
condition and how best to manage it this would be a great fit for the patient to get a good base
understanding that checking his sugars regularly are important and that his diet and weight do
affect his condition and this web page supports this this web page is easy to locate with a
smartphone which the patient has and use as well the third interactive technology is the
wellframe app this is an application where the patient can put his knowledge to the test so to
speak uh and try the available included fitness programs update his weight including his Target
weight goal use the step tra
cker input medications and use reminders to take those medications and
if he wanted to he could communicate with a health care Advocate if there are any complications or
questions along the way this app is available for use for patients but it provides a platform
where patients can keep track of their health success make updates and maintain it this
would be a good fit for the patient because he is technology savy and can download and use
apps well although the Dexcom 7 um is the only inter
active technology with out of the three that
does cost money usually insurance will chip in to cover the cost but one thing I love about the
other resources are they are essentially free um anyone can have access to the internet uh you
can go to your local library um people have cell phones usually um more likely than not um and
then the app the wellframe app is free as well and I think this makes them
more appealing resources to patients so there was success for Joe overall
with the teach
ing and the Technologies in place the patient has had great success he was
really enthusiastic about using the Dexcom G7 and wanted to check his sugar right away the
patient does check his sugar regularly now every day when he needs to or when he's supposed to
his sugar levels have dropped with this use of the continuous glucose monitor and with the
education from the American Diabetes Association web page he states he has a better understanding
overall of how his lifestyle affects his chro
nic condition and he has all of the information he
needs to make healthy changes the patient also states he uses the wellframe app regularly for
medication reminders um he uses the step tracker daily and he hasn't had any needs so far to
communicate with the health Advocate since things are going so well the patient states
he has lost eight pounds in the past month and he is looking forward to his next hemoglobin
A1c results and he states he is feeling better already so this was my
patient
